If you are an authorized representative of a Long Term Care Facility, use this application if:

1. You represent a Skilled Nursing Facility, Intermediate Care Facility, Residential Care Facility, or Assisted Living Facility, and you have uncompensated testing costs (a laboratory billed you for their testing kits or processing services).

2. You represent a Skilled Nursing Facility, Intermediate Care Facility, Residential Care Facility, or Assisted Living Facility, and you have other uncompensated and necessary COVID-19 related costs.
